Key Responsibilities
Initial review, triage, and assignment of patients to case managers
Supervise staff and ensure quality, cost-effective care
Develop collaborative relationships with other departments and community agencies
Conduct assessments of patients' clinical and psychosocial status
Assist in discharge planning and ensure continuity of care
Required Qualifications
Bachelor's Degree in Related Discipline
3 years directly related experience
RN in NM or as allowed by reciprocal agreement by NM
CPR for Healthcare/BLS Provider or Professional Rescuers within 30 days
Preferred Qualifications
Master's Degree
Bilingual in English and Keres, Tewa, Tiwa, Towa, Zuni, or Navajo
