Join MJHS as a Hospice Field Case Manager, where you'll provide compassionate care to patients and their families in both community and facility settings. You'll develop personalized care plans and oversee home health aides, making a meaningful impact in end-of-life care.
Key Responsibilities
Perform physical and psychosocial assessments for hospice patients
Electronically document visit occurrences
Develop and implement nursing plans of care
Contact physicians for care plan changes as needed
Supervise, train, and evaluate home health aides and homemakers
Required Qualifications
Associates/Diploma from an accredited school of Nursing
New York State RN licensure
Valid NYS driver's license
Effective communication and teaching skills
Preferred Qualifications
BSN
At least two years experience in medical-surgical, community health, and/or oncology nursing
Proficiency with Word and Excel
Experience working within an interdisciplinary team environment
