Seeking a dedicated Home Health Occupational Therapist for a 13•week contract in Sacramento, CA. This role involves providing exceptional care with a flexible schedule and no on•call duties.

**Key Details:**

• 40 hours/week
• Must reside within 30 minutes of territory
• Weekly in•office presence required

**Qualifications:**

• CA OT License & BLS Certification
• 2+ years of recent home health experience
• Proficient in HCHB/OASIS, including Oasis E
• Manage 20•25 cases/week, 5•7 productivity points/day
• Documentation completed same day at patient's home

**Additional Requirements:**

• Homecare Homebase experience
• Maximum 1•year gap in experience
• Orientation: 2•3 days
