Key Responsibilities
Adhere to and promote the core expectations of the Patient Centered Medical Home
Ensure quality customer service through training and coaching
Assist patients to access community resources and financial assistance
Collaborate with medical providers and multidisciplinary teams
Document all client encounters in the electronic medical record
Participate in hospital-wide activities and committees
Required Qualifications
Program Graduate
1 year directly related experience
CPR for Healthcare/BLS Provider or Professional Rescuers within 30 days
RN in NM or as allowed by reciprocal agreement by NM
Preferred Qualifications
Bachelor's Degree
BSN or Bachelor's in Related Discipline
