Key Responsibilities
Coordinate patient care systems and services
Conduct timely discharge planning
Develop comprehensive multidisciplinary plans of care
Identify areas for performance improvement
Collaborate with healthcare team members for quality patient outcomes
Monitor and evaluate patient responses to interventions
Required Qualifications
Program Graduate
1 year directly related experience
RN in NM or as allowed by reciprocal agreement by NM
CPR for Healthcare/BLS Provider or Professional Rescuers within 30 days
Preferred Qualifications
Bachelor's Degree
Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo
