Job Title: Home Health Physical Therapist - Days

Seeking a dedicated Physical Therapist for home health care in Sacramento, CA. Deliver exceptional patient care in a flexible, dynamic environment.

Responsibilities:

• Provide in-home physical therapy.
• Document using Homecare Homebase and OASIS.
• Manage 20-25 cases weekly, achieving 5-7 productivity points daily.
• Attend weekly in-person meetings.
• Adapt to patient schedules, including early or late visits.

Qualifications:

• CA Physical Therapist License.
• BLS Certification.
• 2+ years of home health experience.
• Proficient in HCHB/OASIS, including Oasis E.
• Strong organizational and communication skills.

Contract Details:

• 13-week, 40-hour workweek.
• No on-call or overtime.
• Pre-approved time off required.

Additional Information:

• Must live within 30 minutes of territory.
• Flexible scrub color.
• Includes 2-3 days of orientation.
