Key Responsibilities
Initial review, triage and assignment of patients to case managers
Coordinate multidisciplinary patient-centered care
Supervise staff and provide leadership
Develop and implement care plans
Assess and monitor patient needs and resources
Facilitate communication among health care team members
Required Qualifications
Bachelor's Degree in a related discipline
3 years directly related experience
RN in NM or as allowed by reciprocal agreement by NM
CPR for Healthcare/BLS Provider or Professional Rescuers within 30 days
Preferred Qualifications
Master's Degree
Bilingual in English and Keres, Tewa, Tiwa, Towa, Zuni, or Navajo
