Job Description
Join Aveanna Healthcare as a Physical Therapist and make a significant impact on patients' recovery in their homes. This role involves assessing patient mobility, creating care plans, and collaborating with healthcare teams to ensure quality care.
Key Responsibilities
Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function
Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes
Provide a written evaluation and plan of care defining physical therapy needs, goals, progress and expected outcomes
Deliver physical therapy services to comply with prescribed physician orders
Maintain plan of care by timely documentation and modifying goals and treatment as needed
Responsible for discharge planning and educating the patient and caretakers on home exercise plans
Participate in coordination of services with the healthcare team
Document effectively to avoid technical or clinical denials
Supervise other personnel (PTA, HHA) as applicable
Complete OASIS assessments where allowed by state professional practice and regulation
Required Qualifications
Degree in Physical Therapy from an accredited university/program
Current, unrestricted state license as a Physical Therapist in the state of practice
Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures

Experience in Home Health Therapy and completion of OASIS documentation
