Job Description

As an RN Field Clinical Care Coordinator, you will manage a panel of members with complex medical and behavioral needs, providing care coordination to improve their health outcomes. Key Responsibilities

  • Engage members to complete comprehensive needs assessments
  • Develop and implement person-centered care plans
  • Collaborate with internal care teams and community partners
  • Provide education and coaching for member self-management
  • Support proactive discharge planning and manage care transitions
  • Advocate for members and families to ensure their needs are met

Required Qualifications

  • Current and unrestricted independent licensure as a Registered Nurse
  • 2+ years of clinical experience
  • Intermediate level of proficiency with MS Office, including Word, Excel and Outlook
  • Access to a designated quiet workspace in your home
  • Reside in a location that can receive a high-speed internet connection
  • Access to reliable transportation to meet with members and providers
  • Valid US driver’s license

Preferred Qualifications

  • Bachelor’s or Master’s Degree in Nursing
  • Certified Care Manager (CCM)
  • 1+ years of community case management experience
  • Background in Managed Care
  • Experience working in team-based care
  • Ability to utilize an Electronic Medical Record
  • Ability to use online training platforms
  • Demonstrated ability to utilize virtual care platforms
