Job Description
Join Humana as a Field Care Manager in Northern Virginia, where you'll assess and coordinate care for individuals facing substance use disorders. Your role will involve working closely with members and healthcare providers to ensure optimal health outcomes through personalized care and community engagement.
Key Responsibilities
Provide personalized care coordination and crisis intervention
Engage members face-to-face to build trust
Complete required assessments including the Comprehensive Risk Assessment
Coordinate behavioral health, substance use, and medical services
Improve member health literacy and address social needs
Oversee care planning and service delivery as part of the interdisciplinary care team
Facilitate communication among providers and care management staff
Conduct biannual and quarterly face-to-face visits
Required Qualifications
Must reside in Virginia
Active LMHP License in the state of Virginia
Minimum of 1 year of experience working directly with individuals with substance use disorder (SUD)
Case management experience for ASAM levels of care
Ability to travel to region-based facilities and homes
Exceptional oral and written communication skills
Ability to work with minimal supervision
Ability to use electronic medical records
Intermediate to Advanced computer skills
Valid driver's license and reliable transportation
Preferred Qualifications
Case Management Certification (CCM)
Experience with Medicare, Medicaid, and dual-eligible populations
Field Case Management Experience
Experience with pregnant and post-partum population with SUD
Knowledge of community health and social service agencies
Knowledge of ASAM levels of care
Managed Care Experience
Bilingual preferred (Spanish, Arabic, Vietnamese or other)
