Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of clients by developing action plans and providing essential resources for their care. You'll work closely with communities to ensure clients lead safe and independent lives.
Key Responsibilities
Function as a case planner and supply appropriate interventions for client care
Conduct routine community contacts on behalf of the social worker
Evaluate actual living conditions of clients through home visits
Identify appropriate treatment or service providers and facilitate client referrals
Provide structure to clients and assess independent living skills
Perform individual/group counseling based on client needs
Conduct community outreach and establish linkages with helpful organizations
Required Qualifications
BA Degree
Experience in a community or social service group
Or at least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
