💵 Pay Range: $43.27 – $62.50 per hour (based on experience)
We are seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our Home Health Specialty team in Phoenix, AZ. As a case manager, you will play a vital role in overseeing patient care, ensuring high-quality outcomes, and supporting patients throughout their home health journey. This is a dynamic position that combines clinical expertise with care coordination and patient advocacy.
You will manage a caseload of patients in their homes, providing direct nursing care, conducting assessments, developing care plans, and collaborating with physicians, therapists, and other team members to ensure optimal health outcomes.
