This remote position involves collaborating with community partners to support housing placements and ensure compliance with housing regulations, all while offering a flexible work environment.
Key Responsibilities
Support housing providers in complying with housing regulations
Assess needs of homeless individuals and families for permanent housing
Collaborate with provider agencies to identify members qualified for housing
Assist members with housing application processes
Track and obtain housing resources for members
Maintain housing database and document services
Coordinate timely transitions to housing placements
Implement housing tools for prioritizing placements
Facilitate processing of housing applications
Develop action plans for obtaining permanent housing
Required Qualifications
Bachelor’s degree in health care related field or equivalent experience
3+ years experience in servicing adults with serious mental illness (SMI) or in a health care related field
Experience in non-profit housing services
Knowledge of fair housing, tenant’s rights, and SAMHSA evidence based practices
Current state driver’s license
Preferred Qualifications
Experience with Continuum of Care Permanent Supportive Housing and Transitional Housing Programs
HMIS (Homeless Management Information System) experience
