Job Description
Join Charlotte Hungerford Hospital as a Clinician to provide evidence-based clinical services and work collaboratively with interdisciplinary teams. This role emphasizes patient-centered care, ethical standards, and the development of effective treatment plans for adults with dual diagnoses.
Key Responsibilities
Conduct comprehensive bio-psychosocial assessments and establish care plans
Collaborate with interdisciplinary teams for care coordination
Monitor progress of interventions and revise plans as needed
Provide direct patient care including case management and counseling
Maintain timely patient-centered documentation
Participate in administrative and development activities
Support the development of hospital programming to meet clinical needs
Contribute to improved patient experience
Ensure cost-effective utilization of resources
Required Qualifications
Master's Degree in Social Work, Counseling, Marriage and Family Therapy or Related Human Service Field
Previous experience in a healthcare setting
LMSW, LPC-A, LMFT-A, or CADC
Valid Driver's License
Computer proficient
Ability to assess problems and make timely decisions
Excellent communication skills
Knowledge and behaviors to meet age-specific patient needs
Focus on personal, patient/client and unit/staff safety
Understanding of health-related social needs and cultural competence
