Key Responsibilities
Evaluate needs of high-risk members and recommend care plans
Act as liaison and advocate for members and families
Support members with mental/behavioral health issues
Perform home and site visits to assess member needs
Educate long-term care members on health topics and resources
Coordinate service authorization for various services
Maintain accurate documentation in electronic care management system
Provide feedback to leadership on quality of care improvements
Required Qualifications
Master's degree in Mental Health or Social Work
Graduation from an Accredited School of Nursing
2 – 4 years of related experience
Licensed Behavioral Health Professional or RN
Preferred Qualifications
Experience with mental/behavioral health needs
Knowledge of crisis intervention and de-escalation practices
