Job Description
Join HarmonyCares as a Nurse Practitioner (PRN) and deliver high-quality home-based healthcare to patients. As part of an established organization, you'll conduct risk assessments and collaborate with a dedicated team to ensure optimal patient care.
Key Responsibilities
Conduct comprehensive in-home health risk assessments
Take history, examine, and determine diagnoses
Provide written documentation of patient visit per NCQA standards
Take patient vital signs as necessary
Place case management referrals and communicate with PCP as necessary
Communicate with patients, caregivers, agency nurses, and other providers as necessary
Perform all clinical duties while observing OSHA Universal Precautions
Maintain patient confidentiality
Attend required meetings and in-services
Participate in professional development activities and maintain professional licenses and affiliations
Required Qualifications
Active/unrestricted medical license
Active CPR Certification
Board eligible or board certified in family medicine
Outstanding EHR skills
Preferred Qualifications
Geriatric training/experience
Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public
