Key Responsibilities
Function as a case planner and provide appropriate interventions for client care
Develop action plans utilizing existing resources
Conduct routine community contacts and home visits
Evaluate clients for individualized treatment programs
Provide case management and counseling services
Perform community outreach and establish linkages
Required Qualifications
BA Degree with experience in community or social service
At least five years of experience as a Case Assistant
Preferred Qualifications
