This role supports patient-centered care coordination in a dynamic, high-acuity environment. The ideal candidate will bring strong clinical judgment, communication skills, and adaptability to meet system-wide needs. Responsibilities Coordinate patient care across the continuum, ensuring safe and effective transitions Collaborate with interdisciplinary teams to develop and implement discharge plans Advocate for patients and families while navigating complex healthcare systems Document accurately using Epic charting system Float within assigned site and across Legacy Health system as needed Required Qualifications Active Oregon RN license or proof of license application and fingerprint submission Minimum 2 years of case management experience BLS certification CCM certification preferred Ability to complete assigned education modules within one week of assignment start Additional Details No on-call required Saturday shift is part of regular schedule First-time travelers accepted Locals not accepted (must reside 50+ miles from any Legacy facility) Finalized RTO must be submitted with application; no RTO allowed in first week; max 7 days per assignment Mandatory low census cancellation: 1 shift per two-week pay period, with 1.5-hour notice Scrub attire: scrubs or business casual for Case Managers Free parking in unrestricted spaces About The Judge Group Judge Healthcare, a division of The Judge Group, bridges the talent divide for companies and organizations across the healthcare ecosystem, providing resources and specialized roles such as travel nurses, allied health professionals, locum tenens, and more.
