Must provide case logs for the past 12 months.
· Practice Setting: Hospital
· Types of Cases: General Surgical Cases ADULT, General Surgical Cases TRAUMA, Central Line Insertion, Pulmonary Artery (PA)Catheter Placement, Chest Tube Insertion, Vascular Access Devices, Cholecystectomy, and Hernioplasty.
· Credentialing Timeframe: 60-90 days
· Electronic Medical Record (EMR): EPIC
· Certifications Required: Board Certified, Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS)
Advanced Trauma Life Support (ATLS)
· Licensure Required: Interstate Medical Licensure Compact (IMLC)
