Job Description
The Case Manager at The Jewish Board will support adults with serious mental illness, guiding them toward independent living through personalized recovery goals. This role involves managing complex cases, advocating for residents, and facilitating their access to necessary services while ensuring compliance with regulatory standards.
Key Responsibilities
Carries a caseload and practices person-centered and collaborative service planning with residents
Ensures service plans are completed and signed by residents
Evaluates and assesses independent living skills
Advocates on behalf of residents with other service providers
Completes regular and timely documentation of all resident interactions
Participates in intake, assessments, discharge, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
