POSITION SUMMARY: This position provides Wound and Ostomy care to patients in the Wound Healing Center at ECHN, under the direction of the Wound, Ostomy and Continence Nurse (WOCN).# Works collaboratively with the interdisciplinary team.# Communicates, as appropriate, the patient#s clinical condition, responding quickly and accurately to changes in condition or response to treatment.# Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served.# Promotes the profession of nursing and professional practice through collegial support and interactions. Reports to the Director of Care Management. # EDUCATION/CERTIFICATION Must be a graduate of an accredited LPN/LVN program and have and maintain a current Practical Nursing license in the State of Connecticut.# # COMPETENCIES Must be able to communicate effectively in English, both verbally and in writing and possess basic computer skills. Assesses and reassesses pain.# Utilizes appropriate pain management tools and techniques.# Educates the patient and family regarding pain management.# Documentation reflects assessments, interventions and evaluation of effectiveness. Updates individualized plans of care as indicated by assessment and patient response to treatment.# #Collaborates with the RN in the evaluation of the overall plan for effectiveness.# Involves the patient and family in the formulation of goals. Performs delegated aspects of patient care in an environment that optimizes patient safety and promotes positive patient outcomes. Documents the nursing process and responses to care in a timely manner utilizing the appropriate tools. Demonstrates an ability to be flexible, organized and function under stressful situations.# Utilizes critical thinking skills and sound judgment in priority setting and delegation. In collaboration with the physician and WOCN, implements a teaching plan based upon identified learning needs and evaluates effectiveness of learning.# Family is included in teaching as appropriate. Provides support to the WOCN by communicating with patients and families, physicians and nursing agencies in follow up to the patient visit. Demonstrates initiative and responsibility in practice consistent with evidence-based standards.# Pursues personal and professional growth and development.# Serves as a professional role model and mentor. With consultation of the WOCN, schedules product demonstrations and education for the wound care team.# As requested, assists with coordination of wound and Ostomy education for nursing staff. Provides support for special projects including participation in the quarterly Pressure Ulcer Prevalence studies and the Wound Blitz. Performs record management, scheduling of appointments and charge entry in the absence of the Administrative Assistant. ##
