Key Responsibilities
Coordinate social work services across the system and outpatient clinics
Assess risk and intervene clinically for complicated grief and bereavement
Provide clinical guidance and consultation to interdisciplinary teams
Conduct comprehensive psychosocial assessments
Serve as a liaison for palliative care education and outreach
Assist with community outreach and caregiver support program development
Educate palliative care clinicians across the system
Train social work staff on primary palliative care skills
Required Qualifications
Master's degree in Social Work from an accredited college or university
Three years post-MSW experience
Valid license as a Clinical Social Worker (LCSW) in the State of California
Preferred Qualifications
Three years experience within a Palliative Care or Hospice environment
Professional certification in the area of Palliative Care
