Join CenterWell Home Health as an Occupational Therapist and make a meaningful impact by providing personalized rehabilitation services to patients in their homes. You'll help individuals regain strength, mobility, and independence through tailored therapeutic programs.
Key Responsibilities
Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs
Confer with the patient's physician and other health care team members
Provide therapeutic treatment and instruction to patients
Review/expand retraining programs
Consult with other vocational team members
Report patient status and progress
Participate in care coordination and discharge planning activities
Design community reintegration activities
Recommend and/or design special adaptive equipment
Design/train staff and family members to carry out the retraining program
Coordinate/oversee/supervise/instruct Occupational Therapy Assistant and Home Health Aide performance
Accurately document patients' care observations, interventions and evaluation
Required Qualifications
Degree from an accredited Occupational Therapy Program
Current and unrestricted OT licensure
Current CPR certification
Good organizational and communication skills
Valid driver’s license, auto insurance and reliable transportation
Preferred Qualifications
A minimum of six months of occupational therapy experience
Home Health experience
