Join Molina Healthcare as an IRIS Consultant and make a meaningful impact on the lives of individuals with long-term health care needs. In this remote role, you'll partner with community members enrolled in the Wisconsin IRIS Program to help them achieve their personal goals through customized planning and resource navigation.
Key Responsibilities
Meet with IRIS participants at least four times a year
Provide program orientation to new participants
Assist participants in identifying personal outcomes
Document all activities within the IRIS data system
Research community resources for participants
Educate participants on budget management
Develop trusting relationships with participants
Collaborate with other staff to implement plans
Attend monthly team meetings and phone check-ins
Required Qualifications
Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or a related field
1+ year of direct experience in social services
Knowledge of Long-Term Care programs
Ability to work independently
Excellent problem-solving and critical thinking skills
Good time management and prioritization skills
Ability to maintain professional relationships
Comfortable working with diverse populations
Resourceful with knowledge of community resources
Proficient in technology including Outlook, Skype, Teams, PowerPoint, Excel, and Word
Valid driver's license with good driving record
