Job Description

Join Optum as an Associate Patient Care Coordinator to enhance health outcomes by connecting patients with necessary care and resources. This full-time role involves telephonic outreach, appointment scheduling, and maintaining patient relationships in a supportive and inclusive environment.

Key Responsibilities

  • Patient telephonic outreach to schedule patients for health events
  • Collect and verify demographic information and insurance eligibility
  • Timely notification and reminder calls for upcoming appointments
  • Follow-up with patients to reschedule appointments
  • Enter dispositions for all calls in the system
  • Monitor patient call frequency to avoid excessive outreach
  • Use telephonic software for outreach and inbound calls
  • Follow approved call script for outreach calls
  • Support individual providers with scheduling requests
  • Direct and follow-up on patient messages and concerns
  • Meet established daily productivity goals
  • Maintain effective working relationships with patients and staff
  • Other duties as assigned

Required Qualifications

  • High School Diploma/GED (or higher)
  • 1+ years of experience in communicating and providing customer service, both in person and via phone
  • 1+ years of experience with general office procedures
  • Beginner level of proficiency with Microsoft applications

Preferred Qualifications

  • 1+ years of related work experience including data entry
  • ICD 10 and CPT coding experience
  • Prior experience with EMR computer applications
  • Bilingual – Spanish/English
