Join ECHN as a Population Health Nurse, where you'll play a crucial role in improving clinical outcomes for patients with chronic diseases through effective coordination of quality healthcare services.
Key Responsibilities
Review and implement clinical workflows and processes
Identify vulnerable patients for PCMH standards
Support physicians in developing individualized treatment care plans
Collaborate with ECMP physician offices to implement PCMH required processes
Participate in regular team meetings and peer review activities
Required Qualifications
Graduate of an accredited school of Nursing
Current license as a Licensed Practical Nurse in Connecticut
Minimum of five years of nursing experience in a community practice health setting
Preferred Qualifications
Bachelor's degree in an applicable field
Experience working with vulnerable populations
Strong analytical, organizational, and interpersonal skills
