Key Responsibilities
Coordinate clinical workflows and processes within physician practices
Analyze patient data and support clinical staff in implementing quality initiatives
Develop individualized treatment care plans in collaboration with physicians
Ensure adherence to PCMH standards and improve clinical outcomes for patients
Required Qualifications
Current Licensed Practical Nurse in the State of Connecticut
Graduate of an accredited school of Nursing
Minimum of five years of nursing experience in a community practice health setting
Preferred Qualifications
Bachelor's degree in an applicable field
Experience working with vulnerable populations
