We are seeking a dedicated and skilled Outpatient Pharmacist to join our healthcare team in Maple Grove, MN. In this vital role, you will be responsible for performing comprehensive clinical pharmacy services, including patient consultation, medication therapy monitoring, and collaborating with medical staff to ensure the highest standard of patient care. The ideal candidate is proficient in clinical functions, detail-oriented, and an effective communicator.

Key Responsibilities

• Provide direct clinical pharmacy services, including patient monitoring and kinetic drug dosing.
• Consult with physicians, nurses, and other clinical staff on medication therapy, interactions, and efficacy.
• Conduct thorough reviews of medication orders for appropriateness, dosage, frequency, and potential drug interactions or allergies.
• Actively participate in the medication reconciliation process.
• Supervise and oversee the dispensing of medications and IV solutions by pharmacy technicians.
• Perform all other duties as assigned to support organizational goals.

Schedule & Shift Details

• Shift Type: Days
• Shift Schedule: Rotating shifts between:
  • 0800 - 1600
  • 1030 - 1800
  • 1000 - 1400 (4-hour shifts)
• Hours: 4-8 Hour Shifts / 32-Hour Guarantee
• Weekends: Every 4th Weekend
• On-Call: None

Qualifications & Requirements

• Experience:
  • Outpatient or Retail Pharmacy experience is required .
  • Recent experience with EPIC Willow is required .
• Education:
  • Bachelor of Science in Pharmacy or a Doctor of Pharmacy (Pharm.D.) degree is required .
• Licensure/Certification:
  • Active, unrestricted license from the Minnesota State Board of Pharmacy is required .
