Job Description
Join Optum's Senior Community Care team as a Nurse Practitioner or Physician Assistant in Portland, OR. Key Responsibilities
Conduct comprehensive assessments
Manage patients through acute medical situations or chronic condition exacerbations
Ensure accurate and complete documentation
Communicate and collaborate with the interdisciplinary care team
Conduct advanced illness and advanced care planning conversations
Provide counseling and education to patients and caregivers
Coordinate care as patients transition through different levels of care
Review orders and interventions for appropriateness
Function independently with minimal supervision
Required Qualifications
Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (for Nurse Practitioners)
Active and unrestricted Nurse Practitioner license or ability to obtain by start date (for Nurse Practitioners)
Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center (for Nurse Practitioners)
Graduate of an accredited Physician Assistant degree program (for Physician Assistants)
Active and unrestricted Physician Assistant license or ability to obtain by start date (for Physician Assistants)
Board certified by the National Commission on Certification of Physician Assistants (NCCPA) (for Physician Assistants)
Active DEA licensure/prescriptive authority or ability to obtain post-hire
Ability to complete physical requirements of the job
Availability to work 24 hours per month, with 16 hours during off-hours
Access to reliable transportation
Preferred Qualifications
New graduates are encouraged to apply
1+ years of clinical experience in senior living, geriatric medicine, or home care
Experience in meeting healthcare needs of patients with complex behavioral, social, and/or functional needs
Understanding of Geriatrics and Chronic Illness
Understanding of Advanced Illness and end of life discussions
Proficient computer skills including ability to document medical information
