Key Responsibilities
Conduct phone and field outreach to locate and enroll clients into programs
Build relationships and trust with clients and their family/caregiver
Work collaboratively on an interdisciplinary team to discuss care needs
Educate clients about program services and self-help techniques
Advocate for effective recovery-based services
Teach and model symptom management and coping skills
Document in EMR in accordance with policies and regulatory requirements
Required Qualifications
High school diploma or equivalent
Minimum of one year experience in a mental health, substance use treatment program, health care or human services setting
Effective oral/written/interpersonal communication and relationship building skills
Ability to work independently and collaboratively on an interdisciplinary team
Computer literacy (electronic health records, word processing, e-mail, internet research, data entry)
Preferred Qualifications
Experience working with a severely mentally ill, psychological trauma, and/or substance using population
Bilingual skills in English and Spanish
