Key Responsibilities
Conduct Adult/Geriatric assessments including medical history and diagnosis
Work collaboratively with healthcare professionals
Provide services in various venues including home visits
Document assessments according to Matrix guidelines
Required Qualifications
Master’s Degree or commensurate experience
Current RN and NP licensure in state of practice
Board certified by AANP, ANCC or AACCN
Current BLS, ACLS or CPR certification
Strong computer skills
Excellent verbal and written communication skills
Preferred Qualifications
1 year experience as a Nurse Practitioner
Familiarity with employee health/medical record software
