Key Responsibilities
Conduct psychosocial evaluations of patients and families
Provide ongoing counseling during end-of-life journeys
Offer emotional support to patients and families
Collaborate with the care team in accordance with the plan of care
Required Qualifications
MS degree in social work from an accredited school
Current state license as a social worker
2+ years of clinical work experience, preferably in healthcare or hospice
Reliable transportation
Ability to sit, stand, bend, move intermittently and lift at least 25lbs
Preferred Qualifications
Experience in hospice care
Strong communication skills
Ability to work in a team environment
