Key Responsibilities
Perform health histories, physical exams, and participant teaching
Order, interpret, and follow up on lab tests and results
Initiate emergency measures as indicated
Assess, initiate, and coordinate participant care daily
Act as liaison with physicians, nurses, and other healthcare professionals
Participate in the PACE provider call schedule
Document participant changes in the medical record
Evaluate and treat participants in various settings
Required Qualifications
Master’s degree in clinical nursing from accredited Nurse Practitioner program
RN licensure in the state
Family certification from Nurse Practitioner program
3-5 years nursing experience with Nurse Practitioner experience preferred
1 year of experience working with a frail or elderly population preferred
Preferred Qualifications
Training on working with a frail or elderly population
Reliable transportation and valid driver's license
Medically cleared for communicable diseases and up-to-date immunizations
