Collaborate with physicians and staff to provide coordinated, comprehensive, high-quality care to patients
Required Qualifications
Bachelor's Degree in Pharmacy from an accredited School of Pharmacy
Ph.D. in Pharmacy from an accredited School of Pharmacy
Postgraduate Year One (PGY-1) and Year Two (PGY-2) Pharmacy Residency or equivalent experience
Oregon Pharmacist License upon hire
Board Certified by the Board of Pharmacy Specialties within 2 years of hire
Pharmacy preceptor license in applicable state upon hire
2 years of Senior Ambulatory Clinical Pharmacist experience or 5 years of pharmacy practice experience with relevant setting experience
