Job Description

The Case Manager at The Jewish Board supports adults with serious mental illness, empowering them to live independently in the community. They oversee complex cases, advocate for residents, and ensure they meet their personalized recovery goals through a compassionate and culturally competent approach.

Key Responsibilities

  • Carries a caseload and practices person-centered and collaborative service planning with residents
  • Ensures service plans are completed and signed by residents according to regulatory guidelines
  • Evaluates and assesses independent living skills
  • Advocates on behalf of residents with other service providers
  • Completes regular and timely documentation of all resident interactions
  • Links residents to community-based treatment and collaborates with other service providers

Required Qualifications

  • Bachelor of Arts (BA) or Bachelor of Science (BS) degree
  • Experience working with individuals living with serious mental illness, substance use disorders, or trauma

Preferred Qualifications

  • At least 1-year experience in Human Services
