Key Responsibilities
Develops and facilitates complex care management activities
Evaluates member needs via phone or in-home visits
Develops ongoing care plans for high acuity members
Coordinates care between members, families, and providers
Monitors care plans and member outcomes
Provides education to members and their families
Documents member information for compliance
Required Qualifications
Master's degree in Behavioral Health or Social Work
Degree from an Accredited School of Nursing
2 – 4 years of related experience
Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN with BH experience
Preferred Qualifications
Experience working with foster care or the Department for Children and Families (DCF)
