1.General job duties. Types of procedures required. • Collaborative work with the Care Team. • Completion of comprehensive patient assessment including patient status to drive discharge. • Driving stay progression • Completion of discharge planning in a manner that expedites patients transition to the next level of care. • Comprehensive and accurate Epic documentation
2.Equipment utilized in the unit/dept. (Is there computerized charting, medication systems, etc.) • Epic documentation • Cell phone
3. Required skills and minimum years of experience. • Ability to work independently • Ability to identify work that needs to be done and follow through to completion without prompting. • Ability to work collaboratively with the care team. • Ability to complete a comprehensive patient assessment including patient status to drive discharge. • Ability to drive stay progression and complete discharge planning in a manner that expedites patients transition to the next level of care. • Ability to document in a manner that is comprehensive, accurate and timely. • Minimum of 2 years experience in the role of IP RN Case Manager.
4.Specific licensures and certifications/registrations required. (ARRT, CRT, BLS, ACLS, etc) • State of Indiana RN License or compact • Prefer certification in Case Management
5.Shift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?)
8 hour shifts, will be specified on the specific request if there is call. They will participate in the weekend rotation
6.Floating to other units/departments. (If so, to which units/departments?) • Will be assigned primarily to one unit but may need to float to another unit/floor but will only be doing IP Case Management work which is the same regardless of the unit/floor.
7.Bed size/number of rooms for this unit/dept. • Units range from 20-36 beds. Depending on the unit size there may be one or two RN Case Managers assigned to a unit.
8.Nurse to patient ratio of this unit. (nursing only)
9.Dress Code: • Business Casual with white lab coat (no jeans) • Close toed shoes
Or Egg Plant colored scrubs top and bottom.
10.Tell us about the unit/department specific training and orientation program. • Parkview General Orientation (6 hours) • Epic Training (4 hours) • Time with peer for Parkview role specific training (8-16 hours) • Limited assignment with peer oversight (8-16 hours)
