Job Description

Join CenterWell Home Health as a Physical Therapist and play a crucial role in helping patients regain strength and independence from their own homes. You'll provide personalized therapy to empower individuals to overcome physical limitations and improve their quality of life.

Key Responsibilities

  • Plan and administer prescribed skilled physical therapy treatment for patients
  • Test/screen the patient’s physical strengths
  • Develop/implement conditioning/rehabilitation programs
  • Confer with physician and clinical team members
  • Provide Physical Therapy Assistants and Home Health Aide staff with instructions/care plans
  • Document patients' care observations, interventions, and evaluations
  • Report patient's progress to the physician and clinical team

Required Qualifications

  • Degree from an accredited Physical Therapy Program
  • Current and unrestricted Physical Therapy license
  • Current CPR certification
  • Strong organizational and communication skills
  • Valid driver’s license, auto insurance, and reliable transportation

Preferred Qualifications

  • Minimum of one year physical therapy experience preferred
