Job Description
Join Professional Case Management as a Senior Case Manager RN and make a difference in the lives of former Nuclear Weapons Workers. Enjoy the flexibility of creating your own schedule while providing essential in-home care and leading a dedicated team of RN Case Managers.
Key Responsibilities
Direct case management of assigned clients
Assist and collaborate with the regional director and other personnel
Plan, implement, and evaluate care provided
Coordinate and manage client care conferences
Serve as a local on-site clinical resource
Assist with staffing coordination and scheduling
Ensure adherence to Agency policies
Participate in interviewing and training of clinical personnel
Review clinical documentation for accuracy and completeness
Required Qualifications
Graduate of a state approved school of professional registered nursing
Current, unrestricted RN license in the state(s) of practice
Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
Current CPR certification
Preferred Qualifications
BSN preferred
One (1) year of supervisory and/or case management experience preferred
