Join CenterWell Senior Primary Care as a Primary Care Physician, leading a multidisciplinary team to deliver value-based, senior-focused healthcare. This role emphasizes quality care and personalized patient outcomes in a supportive environment.
Key Responsibilities
Evaluate and treat center patients according to standards of care
Monitor patient care quality using data and chart reviews
Coordinate patient services including specialty referrals
Participate in daily huddles with the care team
Assist in setting a cooperative tone in practice
Complete medical record documentation timely
Meet with Regional Medical Director regarding quality of care
Participate in growth opportunities for services
Participate in local primary care on-call program
Ensure compliance with licensing and accrediting bodies
Focus on direct patient care and related administrative tasks
Required Qualifications
Current and unrestricted medical license
Graduate of accredited MD or DO program
Excellent verbal and written communication skills
Skill with interpersonal relationships
Engaged in integrated team-based care
Willingness to learn in a value-based care setting
Superior patient/customer service
Basic computer skills including email and EMR
Preferred Qualifications
Board Certification in Family Medicine, Internal Medicine or Geriatric Medicine
Active and unrestricted DEA license
Medicare and Medicaid Provider Numbers
2-5 years applicable experience
Experience managing Medicare Advantage panel
Knowledge of Medicare guidelines
Bilingual
Knowledge of HEDIS quality indicators
