Position Title: Clinical Staff / Ambulatory Pharmacist
Position Summary
The Clinical Staff / Ambulatory Pharmacist provides comprehensive patient care services across inpatient and ambulatory settings. Core responsibilities include medication order verification, dispensing, and other pharmacy operations that ensure the safe, effective, and efficient use of medications .
This role may include rotations into ambulatory care services such as the Anticoagulation Clinic , Oncology , or other specialty areas as determined by the Pharmacy Manager. The pharmacist will contribute to patient care for a broad range of populations, including pediatric, adult, and geriatric patients.
Minimum Requirements
Education
- Required: Bachelor of Science in Pharmacy
- Preferred: Doctor of Pharmacy (Pharm.D.) degree
Experience
- Recent hospital pharmacy experience preferred
- For ambulatory care rotations: Pharmacy residency training preferred
- Advanced practice certifications are desirable
Knowledge, Skills, and Abilities
- Comprehensive knowledge of current state and federal pharmacy laws and regulations
- Strong communication and collaboration skills to effectively interact with:
- Pharmacy colleagues and technicians
- Physicians, nurses, and other healthcare professionals
- Patients and external representatives
- Commitment to patient safety and continuous quality improvement
Licensure / Certification
- Current licensure as a Pharmacist by the Pennsylvania State Board of Pharmacy , or eligibility for reciprocation within 90 days of hire
- For Anticoagulation Clinic rotations:
- Must meet American Heart Association Healthcare Provider Basic Life Support (BLS) requirements and maintain renewal every two years
Supervision Received
- Functions independently with minimal supervision from the Pharmacy Manager and/or Pharmacy Supervisors
