**Job Description:**

We are seeking a skilled Home Health Occupational Therapist to provide care in patients' homes. This role involves delivering therapy, managing a caseload, and completing documentation using HCHB/Oasis.

**Key Responsibilities:**

• Provide occupational therapy in home settings.
• Document daily using HCHB and Oasis E.
• Handle 20•25 cases weekly, achieving 26•28 productivity points.
• Attend weekly in•person meetings.
• Adapt to different territories as needed.

**Requirements:**

• Licensed Occupational Therapist.
• BLS Certification.
• 2+ years of home health experience.
• Proficient in HCHB and Oasis E.
• Flexible 40•hour workweek.

**Additional Information:**

• 13•week contract, possible extension.
• No on•call or overtime.
• Pre•approved time off required.

**Work Environment:**

• Home•based care across Dallas•Fort Worth.
