Job Description
Join UnitedHealthcare as a Foster Care Coordinator, where you'll use evidence-based approaches to support children in the foster care system. This field-based role allows for flexible telecommuting within Kansas, enabling you to make a significant impact on the lives of children and families.
Key Responsibilities
Assess, plan and implement individualized care strategies for children and youth
Coordinate referrals for psychosocial, behavioral, and medical services
Manage the care plan throughout the continuum of care
Communicate health-related information to ensure quality care
Advocate for children, youth, and families
Provide trauma-informed education to members and caregivers
Coordinate with child welfare and juvenile justice systems
Coordinate additional services including educational advocacy and behavioral health
Required Qualifications
Master’s Degree in Social Work, Nursing, Psychology or related human services area
Unrestricted, independent licensure as a Registered Nurse
2+ years of experience in a community health setting
1+ years of experience working with multi-system children
1+ years of experience with MS Word, Excel and Outlook
Ability to travel in assigned region
Access to reliable transportation & valid US driver's license
Preferred Qualifications
Licensed Social Worker or clinical degree
Background in managing populations with complex medical or behavioral needs
Experience with electronic charting
Experience with arranging community resources
