Job Overview:

We are seeking an experienced Registered Nurse – Case Manager to join a supportive healthcare team on a travel contract. The RN Case Manager will oversee care coordination, discharge planning, and patient advocacy across multiple care settings.

Responsibilities:

Coordinate and implement patient-centered care plans.

Conduct assessments and manage safe, effective discharges.

Collaborate with physicians, social workers, and interdisciplinary teams.

Ensure compliance with all CMS, HIPAA, and regulatory standards.

Requirements:

• Active RN license (ME or Compact).
• Minimum 3 to 5 years of case management experience in acute care.
• BLS certification (required).
• Strong knowledge of utilization management and discharge planning.

Previous travel nursing experience preferred.
