Job Description

Join Monte Nido & Affiliates as a Social Worker and play a vital role in providing care coordination for clients seeking treatment for eating disorders. This hybrid position offers the chance to make a significant impact in clients' lives while working within a supportive team environment.

Key Responsibilities

  • Care Coordination & Case Management: Partner with the clinical team to offer case management and care coordination for clients and families.
  • Community & Family Support: Assist in connecting clients and families with relevant community-based programs and resources.
  • Referral Management: Send referrals to community programs and facilitate collaboration with outpatient providers.
  • Transitions Between Care Levels: Help facilitate smooth transitions between various levels of care.
  • Prepare Clients & Families for Transition: Provide necessary information and emotional support to clients and families.
  • Ongoing Resource Connection: Ensure continuity of care by connecting clients with appropriate resources post-discharge.

Required Qualifications

  • Master’s degree in Social Work or related field
  • Experience with CareOregon/Medicaid
  • Proven experience in case management or care coordination, preferably in behavioral health or residential treatment settings
  • Strong communication skills
  • Ability to work collaboratively with a multidisciplinary team
  • Ability to build relationships with clients, families, and community providers
  • Understanding of available community resources, government programs, and insurance processes

Preferred Qualifications

  • LCSW preferred
