Key Responsibilities
Supply appropriate interventions for client care
Develop action plans to utilize existing resources for client care
Routine community contacts for social worker
Evaluate living conditions of clients through home visits
Run groups under supervision of social worker
Conduct individualized assessments for treatment programs
Perform individual/group counseling as needed
Provide case management and community outreach
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
