Position Summary:

VitalHire is seeking a skilled and compassionate SLP - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Willows, CA. Key Responsibilities:

• Provide speech-language pathology services in skilled nursing facility setting
• Conduct comprehensive speech, language, cognitive, and swallowing evaluations
• Develop and implement individualized treatment plans based on patient assessments
• Provide direct therapy services for speech, language, cognitive, and dysphagia disorders
• Document patient progress and maintain accurate clinical records
• Collaborate with interdisciplinary team members including physicians, nurses, and other therapists
• Participate in care plan meetings and discharge planning
• Educate patients, families, and caregivers on therapeutic techniques and strategies
• Monitor patient progress and adjust treatment plans as needed
• Ensure compliance with state and federal regulations for skilled nursing facilities
• Maintain professional licensure and continuing education requirements
• Work scheduled shift hours from 08:00:00 to 16:30:00
• Complete required documentation within established timeframes
• Adhere to facility policies and procedures

Job Requirements:

• Active, unencumbered license in CA
• Minimum of 1-7 years of relevant experience, depending on specialty
• Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
• Experience with EMR systems (Epic, Cerner, as applicable)

Other Required Documentation:

• Current Resume listing detailed work history (MM/YYYY format)
• Two current clinical references
• State-required professional license
• Physicial exam (completed within the past 12 months)
• TB skin test or Quantiferon Gold (within the past 12 months)
• Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
• Current Tdap and Flu vaccinations
• Must pass background check and drug screening
