We are seeking a motivated Physical Therapist for a 13-week contract assignment in a skilled nursing/long-term care setting. This role involves providing high-quality rehabilitation services, creating individualized treatment plans, and helping patients regain mobility, strength, and independence.

Key Responsibilities:

• Assess, plan, and implement physical therapy programs tailored to each patient’s needs
• Monitor and document patient progress, adjusting plans as needed
• Collaborate with interdisciplinary teams to optimize patient care and recovery
• Educate patients and families on mobility techniques and safe practices
• Ensure compliance with facility policies, procedures, and documentation standards

Qualifications & Skills:

• Must hold an active Physical Therapist license in the state of assignment
• Minimum 1 year of experience preferred
• Strong communication and teamwork skills
• Commitment to patient-centered care and rehabilitation excellence
