Job Description

Join Optum as a Nurse Practitioner or Physician Assistant in Senior Care, providing vital support to patients transitioning from rehab to home. Utilize your expertise to enhance care quality while enjoying the autonomy to work independently in various settings.

Key Responsibilities

  • Perform comprehensive assessments and document findings
  • Ensure all diagnoses are ICD10 coded accurately
  • Address and document all quality elements
  • Conduct initial medication reviews
  • Attend and complete all mandatory training requirements
  • Travel between care sites

Required Qualifications

  • Certified Nurse Practitioner through a national board
  • Graduate of an accredited master’s degree in Nursing (MSN) program or accredited Physician Assistant degree program
  • Active and unrestricted APC license in the State of Connecticut
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire
  • Driver’s license and access to reliable transportation
  • Availability to work 10 hours per week
  • Ability to gain a collaborative practice agreement, if applicable

Preferred Qualifications

  • 1+ years of hands-on post grad experience within Long Term Care
  • Ability to move a 30-pound bag in and out of car and navigate stairs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficient computer skills including electronic medical records
  • Ability to develop and maintain positive customer relationships
  • Adaptability to change
