Unit Overview

Unit Name: Med Surg/Tele – Inpatient Surgical (3rd Floor)
Bed Count: 48 beds (two buildings)
Accepts LOCAL clinicians: ✅ Yes ( pay local only) and Travel ✅ Yes

👩‍⚕️ Staffing & Ratios

• RN:Patient Ratio:
  • Nights: 1:4–6
  • LPNs available
  • Virtual admission/discharge nurses assist
• Model: Primary care nursing (must be comfortable with this)
• Charge Nurse:
  • Days – typically Blessing staff nurse
  • Nights – may be house supervisor (smaller assignment to assist)
• CNA Support: Yes (may be pulled for sitter duty, more need on nights)

🩺 Patient Population

Common Diagnoses & Procedures:

• Neuro: Laminectomy, cervical fusion, LESI
• Surgical: Belly surgeries, ostomies, hernia repairs
• Ortho: Total joint replacements (hips/knees)
• Vascular: Fem-pop bypass, dialysis catheter insertions, BKA/AKA
• Other: DM, sepsis, chronic pain (PCA/epidurals/narcotics), urgent OR cases
• Nutrition: PICC, TPN, lipids
• Dialysis: Located on same floor

💉 Skills & Requirements

Required Certifications:

• BLS
• NIHSS

Must-Have Experience:

• ≥1 year surgical experience
• Total joint experience required
• PCA pump management & reversal agents (Narcan)
• Sepsis management
• TPN/lipid administration
• Familiarity with narcotic protocols

💊 Drips & Monitoring

• Titrated Drips: Heparin (per protocol)
• Tele Monitoring:
  • Draeger monitors
  • Monitor techs available
  • RNs must interpret their own strips

🫁 Respiratory & Equipment

• CPAP (home units)
• Possible Airvo
• Occasional Bipap (stays on unit)
• Managed by RT

📅 Scheduling & Shifts

• Shifts: 3x12s (start at 7)
• Self/Block Scheduling: Self-scheduling allowed; block scheduling not guaranteed
• Schedule Release: 6 weeks in advance
• Weekends: Every other (Sat/Sun)
• Holidays: Rotate with core staff
• Call/Standby: None
• Floating: Like units only (may act as helper if higher acuity)
